Our compassionate staff offers individualized care plans for each resident to ensure personalized care is provided to all our patients with a goal to return home.Our team of qualified professionals provides compassionate care by:Maintaining high medical integrityFostering a team spirit among staffCreating friendly, beautiful surroundings for our residents and their visitorsWe understand the importance of creating a comfortable and nurturing atmosphere, whether residents stay for short-term treatment or long-term care.Job DescriptionAssists in interviewing and obtaining information from residents and families to assist Social Services Director in development of psychosocial history, assessment and care plan upon admission, quarterly and upon change of condition for social service needs.Assists resident and family with transition and adjustment to placement in facility.Assists with determination of legal status of residents upon admission, provides information and obtains copies of advance directives, assures that a resident and family wishes regarding treatment (including Do Not Resuscitate (DNR) and withholding treatment) are implemented in accordance with Company policy.Informs the resident/family of resident's personal and property rights.Provides input on social, psychological, emotional needs of resident/family to supervisor.Ensures residents have required evaluations for visual, audio, mental health and dental needs, and follows up as appropriate.Arranges residents' appointments including transportation.Participate in discharge planning, development and implementation of social care plans and resident assessments and facilitate involvement of the resident and/or his or her representative as able.Participate in behavior management metingsAssist in providing solutions for social and practical environmental problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required.Collaborate with other disciplines to schedule Interdisciplinary team meetings for discharge planning, psycho-social/behavioral care plans, and resident assessments with new admissions, quarterly and annually, or significant change as necessary.Ensure assessments/observation are completed timely with new admissions, quarterly and annually, and with significant changes.Complete appropriate sections of the MDS timely.Report all allegations of resident abuse and/or misappropriation of resident property in accordance with facility policy and State and Federal regulations.Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the resident.Maintain the Theft & Loss program of the facility and store Theft & Loss records per state and federal regulations.Other related duties as assigned by the facility Administrator or Director of Nursing.RequirementsBachelors Degree from an approved school of social work or Bachelors in Psychology, Sociology, or other related fields or SSD Certification2-3 years Social Services experience in a Long-Term Care/ Skilled Nursing setting.PCC experience preferredBenefitsWhy Work for Sunnyside Nursing & Post-Acute CareFull-timeTop of the market wages: $22-$25/ hour based on experiencePaid orientation and trainingOpportunities for growthPaid sick leave/paid holidaysMedical, dental, vision, and AFLACJoin the dream team where teamwork is facilitated through hard work and dedicationWe take great pride in meeting or exceeding CDC and CMS standards.
