Job Description Job Description We are seeking a compassionate and motivated Occupational Therapist (OT) to join our growing Home Health team. The ideal candidate is patient-focused, organized, and passionate about helping individuals maintain independence and quality of life in their homes. Position Type: Full-time / Part-time / PRN Setting: Home Health Responsibilities: Evaluate patients in their homes and develop individualized treatment plans Provide skilled OT services to improve daily living skills and functional independence Educate patients and caregivers on adaptive techniques and safety Document evaluations, progress notes, and discharge summaries in a timely manner Collaborate with interdisciplinary healthcare team members Follow all state, federal, and agency regulations Requirements: Current Occupational Therapist license in good standing Graduate of an accredited OT program Home health experience preferred (but not required) Reliable transportation and valid driver’s license Strong communication and time-management skills Ability to work independently
