- 5 days x 8 hours Contract Duration: 13 Weeks Start Date:
1/19/2026 About the Position The RN Home Health provides skilled
nursing care to patients in their homes, including assessments,
medication management, wound care, and patient education. The nurse
coordinates care with physicians and interdisciplinary teams to
support recovery and promote independence. Requirements
Requirements: Active RN license (state-specific as required)
Minimum 1–2 years of Home Health or relevant clinical experience
BLS certification required Strong assessment, documentation, and
patient education skills Proficiency with EMR/OASIS documentation
