Pay Information $2,440 per week About The Position Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum 2 years in acute medical care and case management License: Active Registered Nurse license Certifications: Relevant certifications in case management preferred Must-Have: Experience with care coordination and discharge planning Description: Weekend coverage is required on a rotating basis. The role includes managing a case load of up to 25 patients. The candidate must have experience with the charting system Epic. Knowledge in community health resources and the Medicare guidelines is essential. Experience in assessing patients and coordinating care is required. The candidate should be competent in the utilization of admission criteria and guidelines. A strong understanding of behavioral health, discharge planning, and prior authorizations is necessary. The candidate must assist in developing treatment plans and coordinating with interdisciplinary teams. Experience with home health, hospice, and long-term care settings is preferred. Ability to organize family meetings and discussions is crucial. The candidate should demonstrate proficiency in managing complex patient cases and medical necessity determinations. Must be capable of performing tasks independently related to durable medical equipment and ambulance transport arrangements. Flexibility to float between facilities within the same state may be required. Participation in holiday shifts is expected. References should include supervisors from comparable settings within recent employment history. Experience with Medicare, HMOs, and insurance-related case management is essential.
