A company is looking for a Care Manager RN - Specialty
Pharmacy.
Key Responsibilities
Manage assigned and pended worklists
Conduct inbound/outbound patient calls for clinical
intervention
Develop, implement, and monitor personalized care/service plans
Required Qualifications
Active RN license (compact license required)
2+ years of clinical experience in acute, community, or clinical
setting
1+ years of case management experience in managed care
Experience in specialty pharmacy
Preferred: Associate's or Bachelor's Degree in Nursing (BSN)
